How do I e-sign a document or consent form?

Here's how to consent documents sent by your clinician in Luminello.  

1. From your main dashboard, click on "Documents."

2. Click on the document you want to e-sign/consent. 

3. To e-sign/consent, add your name under the "Signature (required)" field, and click "Complete." 

PDFs

1. To e-sign/ e-consent to PDF documents, check the "I have read and agree to the terms in this document" box. 

2. Click "Sign" to e-sign/consent to the PDF document.

Note: Not all clinicians offer this feature to their patients/clients. Please message your clinician if you do not see the "Documents" tab on your account.
